To all persons or entities in the United States who received a telephone facsimile message on a telephone facsimile machine from or on behalf of Optum360 regarding the Optum360 Essentials Coding, Billing and Compliance Conference on April 15, 2015, May 21, 2015, June 17, 2015, July 23, 2015, August 13, 2015, September 16, 2015, or October 9, 2015.

October 13, 2019: To receive a share of the settlement funds, you must complete and return a signed Claim Form, postmarked, faxed, or uploaded to the settlement website by this date.
September 13, 2019: If you want to be excluded from the class, you must mail your exclusion request so that it is postmarked by this date.
September 13, 2019: If you are a member of the class and you do not believe that the proposed settlement is fair, adequate, and reasonable, your written objection must be filed and received by the Clerk of Court by this date.
November 20, 2019: The Court will hold a hearing to decide whether the proposed settlement should be approved at 1:30 p.m., in Courtroom 3G of the U.S. District Courthouse, 601 Market Street, Philadelphia, PA 19106.
